MANDATORY EXAM PREP:

All appointments are subject to MANDATORY preparatory protocols. If these protocols are not followed, the appointment will be cancelled and a $50 non-refundable fee charged. No exceptions. Be sure to review these protocols to insure compliance, they will be emailed to you approximately 1 week prior to your appointment.

CANCELATION POLICY:

48 hour notice is REQUIRED. Text, Email, or call the clinic 48 hours prior to your appointment to cancel/reschedule.

Failure to do so will result in a $50 fee and inability to reschedule until the $50 fee is paid.

If you have cancelled or rescheduled 3 times, all future appointments are subject to FULL PREPAYMENT regardless of the situations.

 

BREAST SCREENING PATIENTS:

Per our interpreting thermologist, only natural breast tissue can be assessed for pathologic risk.

If you have had complete bilateral mastectomy, we will NOT perform any chest imaging. 

Patients with implants in addition to natural breast tissue ARE still eligible for thermographic screening.

Payments types accepted at time of service: Cash, Check, Visa, Mastercard, Discover, American Express and some HSA/FSA debit cards. In the event the HSA/FSA card declines, you are responsible for full payment and we will provide an itemized receipt for reimbursement.
